What does Support Coordination Consist of:
ASSESSING the needs wishes and desired outcomes of each individual.
DEVELOPING an Individual Support Plan (ISP) with the assistance of the individual their family and those identified as part of the team.
LOCATING the supports and services identified in the ISP so as to meet the needs of the individual and their family.
COORDINATING the identified supports and services as requested by the individual and their family.
MONITORING supports and services to assure that the ISP is implemented. that the ISP continues to meet the needs and desires of the individuals and that the health and safety of the individual is assured.
MAINTAINING eligibility information and assisting individuals to apply for assistance for which they are eligible.
ADVOCATING for the needed supports and services not readily available to the individual.

Adult Day Health Care (ADHC) is a non-residential program that can be an alternative to nursing home care for people who do not need care 24 hours a day, 7 days a week.

Early Steps Program provides intervention services to children with a developmental delay from birth to age 3
Children's Choice Waiver offers supplemental support to children through age 18 with developmental disabilities who currently live at home with their families or with a foster family.
New Opportunities Waiver program includes an array of services aimed at assisting people to live independently as possible. Age 3 years and older and have a developmental disability, which manifested prior to age 22.
Supports Waiver provides options and meaningful opportunities through vocational and communities inclusion for individuals who are 18 years of age and older and meet the eligibility criteria for waiver services.
Residential Options Waiver (ROW) offers individuals of all ages, services designed to support them to move from ICFF?DD and nursing facilities to community-based settings and to serve as an alternative to institutionalization.
Outpatient Mental Health Services provide individual, family group therapy to help adults, teens, and children build on their strengths and improve their quality of life.
